Special Olympics Iowa Release and Medical Forms

*Please Note: Special Olympics Iowa has transitioned to updated Release and Medical Forms effective October 1, 2017. Any athlete with a current Physical and Consent Form on file will NOT need to renew with the new forms until it expires. 

Before athletes can begin competition in Special Olympics Iowa, they must submit the following forms:

A current Physical and Release Form must be on file with the Special Olympics Iowa state office in order for an athlete to be eligible to practice and compete. These are the only physical and release forms we can accept.

If you are unsure of the date your athlete’s Physical and Release Forms will expire, please email physicals@soiowa.org or call 515-986-5520.

Athlete Information Form

One page that needs to be submitted every three years with the Physical Exam. This form gathers basic information and demographics on the athlete. These sections must be filled in completely.

Participant Release Form

One page that only needs to be filled out once for an athlete. This is a legal document that must be submitted one time unless an athletes’ name legally changes. If either box is marked under #4 Emergency Care, then an Emergency Care Refusal Form must be requested from the state office and completed.

Athlete Medical Form- Health History 

The Health History is important for two reasons; To have medical information on hand during training and competition, in case of a medical emergency, and to ensure that the physician is informed of the athlete’s health history when performing the exam.

Athlete Medical Form- Physical Exam

Indicates if the athlete has been cleared to participate in Special Olympics sports. Must be completed by a licensed health professional and needs to be renewed every three years. If an athlete requires further examination due to a concerning health issue before clearance can be determined, a referral form is available.

After Completing the Required Forms 

Make a copy of the Physical and Release Forms and send it to Special Olympics Iowa, PO Box 620, Grimes, Iowa 50111, or email physicals@soiowa.org. If the copy is not legible, we may ask for the original copy. In this case, please make sure to keep a copy for yourself. Once it is received, the state office will then validate the Physical and Release Forms. The Physical and Release Forms will not be validated until all information is correct and completed on the approved form.

Special Olympics Iowa requires that all Physical and Release Forms be submitted to the state office prior to the submission of  area and/or state registration forms. All forms for the event in question must be valid throughout the completion of that competition.

Forms not on file or in receipt of by the specified deadline will not be accepted.
